Summary
This study tested whether a 90-day support program could help people recover from sepsis and prevent them from being readmitted to the hospital or dying. The program used a nurse to coordinate care, check on patients, and help manage medications and other health needs after they went home. Researchers compared this program to usual care in nearly 4,000 high-risk sepsis survivors across eight hospitals.
