Summary
This study tested whether a digital home monitoring kit, including a wearable device and a care plan app, could help people with heart failure take their medications more effectively. It compared the digital program plus usual care against usual care alone over 90 days. The goal was to see if remote monitoring could improve treatment and reduce hospital visits.
