This study gives 400 adults with diabetes or high blood pressure free home-delivered produce boxes and vouchers for 6 months, plus nutrition education and self-management classes. The goal is to see if this helps them eat more fruits and vegetables, feel less food insecure, and improve their health. Participants must be low-income, food insecure, and receiving care at Rush University Medical Center.
