Can weekly calls after prison stop a health crisis?
NCT ID NCT06794983
Summary
This study is testing whether a support program helps people connect to regular doctors after leaving prison. It focuses on adults with a history of substance use and chronic health conditions. Researchers will compare if weekly check-in calls from a nurse case manager lead to more doctor visits than usual care alone.
