NCT ID NCT06611228
First seen Jun 27, 2026 · Last updated Jun 27, 2026
Summary
This study tests whether a combined program of exercises, education, and technology support can prevent disability in older adults (over 70) who are already frail. About 288 participants will either receive this program during their hospital stay and at home for 3 months, or get usual care. The goal is to see if the program improves physical function and reduces hospital readmissions.
