Summary

This study aims to find the best way to prevent future falls and fractures in older adults recovering from a broken bone in a nursing facility. It will compare three different remote care models delivered by a nurse consultant and a team. One model focuses on reducing medications that increase fall risk, another on treating bone weakness (osteoporosis), and a third combines both approaches.
