New program aims to stop kidney patients from bouncing back to hospital

Summary
This study is testing a new follow-up program for adults who have just recovered from a serious episode of sudden kidney injury (AKI). The program provides extra support from a care team after leaving the hospital. Researchers want to see if this extra help keeps people healthier at home, reduces their need to return to the hospital, and prevents further kidney damage.
