Summary
This study tested a patient-oriented discharge summary (PODS) – a one-page tool filled out during hospital stay – to help older patients and their caregivers remember key information after leaving the hospital. 111 older adults with multiple health conditions were enrolled. The goal was to see if the PODS improved the quality of the transition from hospital to home and reduced problems after discharge.
