Summary

This study tests whether a personalized weaning plan can help adults with long-term health problems (like heart disease or COPD) get off breathing machines sooner. About 1380 participants will either receive standard care or a tailored approach that identifies the main reason for weaning difficulty (e.g., lung, heart, or muscle issues) and targets treatment accordingly. The goal is to increase the number of days alive and free from the ventilator within 28 days.
