Summary

This study tested whether having a clinical pharmacist on the hospital team helps prevent medication errors in patients with kidney problems. Researchers compared three groups: one with no pharmacist, one where doctors got extra training, and one where a pharmacist actively reviewed medications and made recommendations. The goal was to see if this extra support leads to safer and more effective medication use for these high-risk patients.
