Summary
This study is testing whether a structured care program led by community pharmacists can help people with type 2 diabetes manage their condition better than their usual doctor's care. It will involve 600 Albertans whose blood sugar is not yet at a healthy target. The goal is to see if easier access to a pharmacist for education, medication adjustments, and health monitoring leads to better blood sugar control and lowers the risk of future heart and kidney problems.
