Summary

This study tested a one-year program to help older adults better manage their type 2 diabetes from home. Over 500 participants received phone calls from nurses to set personal health goals and were connected to local community resources. The goal was to improve daily life and reduce risks like low blood sugar by providing more personalized, ongoing support.
