Summary
This study is testing a program called PROMOTE, which combines peer support from trained community health workers with remote monitoring of blood sugar levels. It aims to help 200 Black adults in Alabama who have uncontrolled type 2 diabetes better manage their condition. The goal is to see if this combined support approach leads to meaningful improvements in blood sugar control and other health measures over six months.
