Summary
This study tested a peer support program for young adults (ages 17-25) with type 1 diabetes as they move from pediatric to adult healthcare. Trained mentors, who also have type 1 diabetes, provided guidance and shared their own experiences over 12 months. The main goal was to see if this support helped improve blood sugar control and made the transition to adult care easier.
