Summary

This study tests whether a team-based approach improves care for frail Veterans having elective surgery. Experts from surgery, anesthesia, geriatrics, and other fields will meet to create personalized care plans for each patient. The goal is to reduce deaths, hospital readmissions, and time spent away from home after surgery by better coordinating care and aligning it with patient goals.
