Inclusion Criteria: * being followed in an outpatient geriatric clinic * Aged 60 years and older, with or without an informal caregiver * Have the cognitive capacity to provide informed consent; * Have no contraindications to physical activity; * Are not currently receiving rehabilitation services; * Are able to communicate, read, and understand French or English; Exclusion Criteria: * None
