Summary

This study tests a new computer tool that helps doctors decide when to refer heart failure patients to palliative care. The tool uses a risk score to predict who might benefit from extra support. Researchers will see if doctors find the tool easy to use and if it leads to more timely referrals. About 2,260 patients and their doctors will take part.
