This study looks at 600 older adults in Singapore to see if early screening for frailty and loss of physical/mental abilities can help them stay independent longer. Participants get check-ups and personalized referrals (like doctor visits or rehab). One group gets help right away, the other waits a year. The goal is to see if early action keeps people moving and living well.
