Summary
This study is testing a program called OPTIMIZE, which uses a detailed checklist and a personal support guide (patient navigator) to help ensure African American pregnant women receive recommended medical and social care. It compares this program to standard care across about 20 community health clinics in Chicago. The goal is to see if the extra support leads to better health checks, breastfeeding rates, mental health, and vaccination uptake during pregnancy and after birth.
