This study looks at whether giving patients one extra hour of breathing support from a ventilator, after they pass a breathing test, helps them stay off the machine for good. About 4,000 ICU adults who have been on a ventilator for more than 72 hours will be randomly assigned to either immediate removal or one more hour of support. The goal is to see if this simple change reduces the chance of needing the ventilator again or dying within a week.
