Summary
This study aims to find undiagnosed or poorly controlled high blood pressure and glaucoma in adults aged 40 and older living in New York City affordable housing. Researchers will screen over 7,000 people and test if a virtual assistant that sends reminders and health information helps more people attend important follow-up doctor appointments. The goal is to improve early detection and connect people to care for these 'silent' conditions.
