New program aims to bridge healthcare gap for men after prison

Summary

This study is testing whether a special nurse support program helps formerly incarcerated Black men manage their type 2 diabetes. One hundred men will be randomly assigned to receive either standard diabetes information or the new program, which includes phone-based coaching on diabetes skills, healthcare navigation, and basic needs support. The main goal is to see if this tailored support improves blood sugar control and quality of life over six months.
