Summary

This study tested whether using mobile monitoring devices and getting support from nurses could help people better manage their type 2 diabetes and high blood pressure. It involved 220 adults whose conditions were not well controlled with standard clinic care. Researchers compared two approaches over a year: one where people just used the devices themselves, and another where nurses used the device data to help guide medication and self-care during telehealth visits.
