Summary
This study tested a nurse-led smartphone app to help heart failure patients manage their health at home after leaving the hospital. 104 patients were split into two groups: one got the app plus regular care, the other got regular care alone. The app provided medication reminders, symptom tracking, and education, with a care team monitoring their data. The goal was to see if the app improved patients' ability to manage their condition and reduced symptoms.
