Summary
This study tested a new support program for people with heart failure after they leave the hospital. A nurse-led team used a smartphone app to help 104 patients track their weight, symptoms, and medications at home. The goal was to see if this extra digital support, compared to standard follow-up care alone, could help patients manage their condition better and improve their health over three months.
