Summary

This study tested whether regular meetings between different types of doctors could work well in a hospital to help patients who have diabetes along with other long-term health problems. About 55 adults with diabetes and at least one other condition (like heart, kidney, or lung disease) took part. The goal was to see if these team meetings were practical and useful for both patients and doctors, not to directly treat the diseases.
