Summary
This study is testing two different ways to provide follow-up care for people who have finished treatment for early-stage breast or colorectal cancer. It compares the usual cancer specialist care to a new model where a primary care doctor works right in the cancer clinic. The goal is to see if the new approach helps survivors get better preventive care, feel more confident managing their health, and avoid unnecessary medical visits.
