Can a simple score help ERs prioritize which seniors need a medication check?

Summary

This study aims to see if a quick frailty screening tool (called the ISAR score) can help predict which elderly patients visiting the emergency room are most at risk for medication problems, like unsafe drug combinations or inappropriate prescriptions. Researchers will collect medical information from 300 patients aged 75 and older to check for a link between their frailty score and these medication risks. The goal is to see if this score could help pharmacists prioritize which patients need their medications reviewed most urgently, but this study itself will not change any patient's care.
