Summary

This study looked at whether carefully managing medications for older adults during their hospital stay and after they go home can lower the chance of being readmitted within 30 days. About 109 elderly patients took part, with one group receiving extra medication reviews and the other getting usual care. The goal was to see if this approach helps keep patients healthier and out of the hospital.
