Summary
This study looks at whether a planned hospital stay that screens and treats all of a patient's health problems (like high blood pressure, diabetes, and weight issues) can improve their heart health. About 70 adults at high risk for heart attacks or strokes will take part. The program gives each patient a personalized care plan and tracks their progress using a simple 7-point health score.
