Summary
This study will test whether an artificial intelligence (AI) system can help community doctors better manage patients who have high blood pressure along with diabetes or high cholesterol. About 6,800 adults will be split into two groups: one where doctors use the AI tool to guide treatment and follow-up, and another where they receive usual care. The goal is to see if the AI approach helps more patients reach healthy targets for blood pressure, blood sugar, and cholesterol.
