Summary
This study tests whether an AI-powered tool can help community doctors improve control of high blood pressure, diabetes, and high cholesterol in 6,800 adults. Participants will be randomly assigned to either AI-assisted care or usual care. The goal is to see if the AI helps more people reach target levels for blood pressure, blood sugar, and cholesterol, reducing their risk of heart disease.
