Summary
This study is testing whether a kidney health coaching program can help delay the need for dialysis or death in Black patients with advanced chronic kidney disease. The program involves six months of support from coaches who have personal experience with kidney disease. Researchers want to see if this personalized support helps patients manage their condition better and stay healthier longer.
