Coaching program aims to keep kidney patients off dialysis

Summary
This study is testing whether a kidney health coaching program can help delay the need for dialysis or death in Black patients with advanced kidney disease. Participants will receive six months of coaching from trained coaches who also have personal experience with kidney disease. The goal is to see if this support helps patients manage their condition better and avoid or postpone more serious treatments.
