Summary

This study tests new ways to help rural primary care clinics provide better follow-up care for cancer survivors. Researchers will give clinics telementoring, quality improvement coaching, and an obesity management toolkit. The goal is to see if these tools help clinics follow national guidelines for cancer survivorship care, such as screening for distress and monitoring for cancer recurrence.
