Summary
This study tests a program to help doctors better screen for chronic kidney disease (CKD) in people with type 2 diabetes. Researchers will enroll 750 adults who are missing key kidney tests. The goal is to see if the program increases the number of patients who get the recommended screening, so kidney problems can be caught and treated earlier.
