Summary
This study tests whether adding visits with a nurse, dietitian, and pharmacist to standard care helps more heart attack patients reach their LDL (bad) cholesterol goals. About 230 adults who had a recent heart attack will be randomly assigned to usual follow-up or extra team-based support. The goal is to see if the team approach helps more patients achieve recommended LDL levels one year later.
