Summary
This study tests a nurse-led program called INFA that screens frail older adults (age 65+) in the hospital for issues like falls, confusion, and malnutrition. Based on the screening, nurses start simple treatments and refer complex cases to a geriatric team. The goal is to see if this approach helps patients maintain their ability to do daily activities six months after leaving the hospital, compared to usual care. About 300 participants are enrolled from medical and surgical wards.
