Summary

This study tested two ways to improve kidney disease care for Hispanic/Latinx and Black patients with diabetes. First, community groups helped screen people for kidney disease and connect them to a primary care doctor. Second, clinics got support to help doctors screen and treat these patients. The goal was to slow kidney disease and learn how to expand these efforts.
