Can a website and a coach help cancer survivors plan End-of-Life care?

Summary
This study tests whether a patient navigator (a health coach) and a website called PACT help blood cancer survivors who had a stem cell transplant plan their future medical care. About 40 adults who are 1 to 5 years post-transplant will be assigned to usual care, the website, the coach, or both. The goal is to see if these tools are practical and acceptable, and whether they help people complete advance directives and have conversations about their care wishes.
