Summary
This study is testing a new monitoring program for veterans with inflammatory bowel disease (IBD), a chronic gut condition. The program helps patients track their symptoms and share updates with their care team between clinic visits. Researchers want to see if this closer monitoring improves patients' quality of life and reduces disease flares and hospital visits over a year.
