Summary

This study tests a program called I-CONNECT for adults 65 and older who have depression or bipolar disorder plus at least two chronic physical conditions like diabetes or high blood pressure. The program assigns a care coordinator who works with the patient's doctor, psychiatrist, pharmacist, and specialists to create a personalized care plan and provide regular follow-up. Researchers will compare this coordinated approach to usual care to see if it improves physical and mental quality of life, care experience, and reduces hospital visits.
