Summary
This study is testing whether a new decision-making guide helps hospital doctors better manage chronic medications like those for diabetes and high blood pressure in older patients. Researchers will see if doctors using the guide feel more confident and make fewer unnecessary medication changes when patients go home. They'll also check if patients understand their medication changes better after their doctor uses the guide.
