Summary
This study is testing a new support program for adults with type 2 diabetes who are leaving the hospital. The program uses a computer survey to identify social challenges like housing or food insecurity and then pairs patients with a helper for 90 days. Researchers want to see if this extra support helps people manage their diabetes better and reduces their chances of needing to return to the hospital.
