Summary
This study is testing whether having a doctor visit patients at home helps manage high blood pressure and type 2 diabetes better than standard clinic visits. Researchers will compare health outcomes for 100 adults who receive two home visits versus those who get usual clinic care. The goal is to see if this more personal approach improves blood pressure control and diabetes management.
