Skipping the hospital: could home care keep dementia patients healthier?

Summary
This study is testing whether providing hospital-level care at home is better than usual hospital care for people with moderate to severe dementia. It will involve 200 participants who have been hospitalized recently and live with or near a family caregiver. The main goal is to see if home care helps patients spend more days at home, reduces hospital readmissions, and improves their quality of life.
