Home coaches aim to keep COPD patients out of hospitals

Summary
This study is testing a home-based coaching program to help adults with COPD better manage their condition. Health coaches will work with 300 participants, primarily from minority and low-income communities in New York City, to improve medication use, exercise, and spot early signs of flare-ups. The goal is to reduce symptoms, improve daily function, and lower emergency room visits and hospitalizations over 12 months.
