Summary

This study tests a home-based cardiac rehabilitation program delivered through a custom app for people who have had a heart valve procedure. Participants who choose not to attend center-based rehab are randomly assigned to usual care, the app with self-guided content, or the app with added video calls from an exercise physiologist. The goal is to see if the app helps improve physical activity, quality of life, and reduce hospital visits.
