VA tests bridge program to get frail veterans into heart recovery at home

Summary

This study is testing a new 'Transition to Cardiac Rehabilitation' (T2CR) program designed to help Veterans with heart disease and other chronic health problems start home-based recovery. The program provides in-person coaching and planning before patients begin standard home-based exercises. Researchers will compare 250 Veterans who get the new T2CR program to those who receive usual care, looking at improvements in physical function, participation in rehab, and time spent living healthily at home over one year.
