Summary
This study is testing a web-based tool called HELPix Care Plan to help parents of children with medical complexity understand and follow discharge instructions. Researchers will enroll 282 parents to see if the tool reduces errors in comprehension and adherence, and lowers the chance of hospital readmission or other problems within 30 days after discharge.
