Summary
This study is testing whether a team of different healthcare specialists can help older adults with blood cancer prepare for and recover from a stem cell transplant. The team includes doctors, therapists, a dietitian, and a social worker who work with patients for three months before and after the procedure. The goal is to see if this coordinated support helps patients stay stronger, avoid complications, and maintain their independence during recovery.
