Summary
This study follows 2,000 adults aged 45-75 in Pennsylvania to better understand why some groups have higher heart disease risk than others. Researchers will test a community-based behavior change program in 800 high-risk participants to see if it works better than usual care. The goal is to improve risk prediction and find ways to reduce heart problems for everyone.
